Caring for a wound that is slow to heal can continue at home, with suitable equipment and guidance from the treating clinician. AOTI TWO2 delivers oxygen locally to the wound area inside a dedicated chamber surrounding the leg. It combines oxygen, cyclical pressure and optional humidification as part of an overall wound-care plan. Contact BIOMED Serres about rental options and the equipment required.
AOTI TWO2: Supporting Wound Care with Topical Oxygen
When a wound remains open for a long time, the treating clinician may consider additional ways to support healing. One option is AOTI TWO2 topical wound oxygen therapy, which has been studied in diabetic foot ulcers and chronic venous leg ulcers.
How Does Treatment Work?
The leg is placed inside a dedicated limb chamber, where the system delivers oxygen and controls cyclical changes in pressure. Chamber humidification is also available. The clinician determines the settings and treatment schedule according to the patient’s needs.
Fitting Treatment into Everyday Life
Following medical approval and equipment training, treatment can take place in the comfort of home. The patient remains seated or lying down and can read or watch television during the session.
- Local application: oxygen is delivered inside the chamber surrounding the affected limb.
- Two chamber sizes: selected according to the limb and wound location.
- Home use: under medical direction and after equipment training.
- Part of ongoing wound care: wound management and clinical monitoring continue as directed by the treating clinician.
What Is Needed to Get Started?
The first step is to discuss with the treating clinician whether TWO2 is suitable for the wound. Treatment requires the controller, a compatible medical oxygen source and single-use chambers.

Clinical Outcomes Reported for TWO2 Therapy
The findings below come from different studies involving specific patient populations, comparators and care protocols.
Diabetic Foot Ulcers
- Complete healing at 12 weeks: 41.7% versus 13.5%. A randomised, double-blind trial of 73 patients, with optimal standard care in both groups.
- The “6 times” figure refers to an ulcer-severity-adjusted odds ratio of 6.00, rather than a sixfold healing rate.
- Recurrence at 12 months: 6.7% versus 40%. This represented 1/15 versus 2/5 healed ulcers. The subgroup was small and the difference was not statistically significant (p=0.070).
Hospitalisations and Amputations
- 88% relative reduction in hospitalisations: 6.6% with TWO2 versus 54.1% without TWO2 within 360 days.
- 71% relative reduction in amputations: 12.1% versus 41.4% over the same period.
These findings come from unmatched cohorts in a retrospective study of 202 patients with diabetic foot ulcers. They show an association and do not establish that TWO2 alone caused the differences.
Chronic Venous Leg Ulcers
- 57 versus 107 days: median time to complete healing with TWO2 versus conventional compression dressings.
- Approximately 87% lower relative recurrence: 3/51 (5.9%) versus 14/30 (46.7%) healed ulcers during 36-month follow-up.
- Pain score reduced from 8 to 3 by day 13: a change recorded in the TWO2 group.
The venous ulcer findings come from a comparative, non-randomised study of 132 patients.
Study outcomes do not guarantee individual results. Treatment is provided within a medically directed wound-care plan.
